

Who We Are
Hospice & Palliative Care Buffalo (HPCB) cares for individuals suffering from serious illness, as well as their families and caregivers. We provide an integrative, holistic (physical, emotional, social, and spiritual) approach to care that focuses on the whole person rather than on individual symptoms. We are here through a continuum of care, spanning the course of the disease through bereavement. Our knowledgeable, compassionate staff helps patients make choices about their care, support those who are grieving, and educate the professional and lay communities on optimum end-of-life experiences.
Earning Hospice Honors Elite status for the past 3 years, HPCB is one of the largest and well-respected non-profit hospice and palliative care organizations in the nation, caring for approximately 1000 patients daily, regardless of age, race, gender, setting, diagnosis, or ability to pay.
At Hospice & Palliative Care Buffalo, we’re here through any long-term or life-limiting illness, providing care for them, and comfort for you.

Our Affiliate Agencies
Hospice & Palliative Care Buffalo (HPCB) is the parent organization over five community-based agencies. HPCB is one of the largest and well-respected non-profit hospice and palliative care programs in the nation.
Hospice Buffalo
Hospice Buffalo cares for seriously ill people with a prognosis of 6 months or less to live. Each patient receives customized, comprehensive care from an interdisciplinary team of healthcare professionals: physician, nurse, social worker, grief support counselor, and volunteer. Hospice Buffalo also offers a respite program for caregiver relief — the patient can stay for up to 5 days at our Inpatient Unit in Cheektowaga.
Essential Care for Children
Essential Care for Children is a home-based palliative and hospice care program that provides coordination of medical services, as well as psycho-social, emotional and spiritual support to seriously ill children and their families.
Home Connections - Adult Palliative Care Program
Home Connections provides home-based, case-managed care and support to patients facing serious, progressive illness and their families. We are here to help patients and families navigate through the course of serious illness, regardless of changes in diagnosis or curative treatment.
Wilson Support Center
Grief support and education that is sensitive, compassionate and responsive to the needs of the WNY community. Services are provided by licensed healthcare professionals and peer volunteers.
Hospice Foundation
Provides support for Hospice & Palliative Care Buffalo’s affiliate agencies and the operational needs of each corporate component to ensure financial stability, as well as funding future programs:
- Helps underwrite operational shortfalls of any affiliate corporations
- Raises funds for new program development and on-going support
- Raises capital and other permanent and temporarily-restricted funds
- Raises endowment funds to ensure long-term sustainability for hospice and palliative care services
- Generates awareness in the community about hospice and palliative care services and programs
