Schofield Care

Schofield Care's mission is to provide the highest level of professional health care and supportive services to frail elderly and disabled individuals. We offer a variety of services offered in both residential and community settings, including long-term care and short-term rehabilitation in our nursing facility, a certified home health care program and an adult day health care program.

The Schofield Residence offers long term care and short term rehabilitation in our 120-bed Nursing Facility in the Town of Tonawanda, NY.

Schofield's Adult Day Health Care Program provides a middle road between living independently and living in a skilled nursing facility. Now located at the corner of Louisiana and Fulton Streets in Buffalo, the program provides health services for people who live in the community aimed at maintaining or improving participants' everyday functioning.

Schofield Certified Home Care is a Certified Home Health Agency (CHHA) that serves Erie and Niagara counties offering comprehensive home health care services include the hightest level of Skilled Nursing; Physical, Occupational and Speech Therapy and Medical Social Services.
